Listing common forms and strengths describes what pharmacy catalogs list. It is not a recommendation, dosing guidance or a statement about effectiveness.

Sourcing considerations for compounded NAD+

Clinics sourcing compounded NAD+ should confirm the points below with the pharmacy before ordering.

  • 503A only: NAD+ is a Category 1 substance under FDA's interim policy, so it is compounded under 503A for identified patients; do not assume 503B or office-use availability.
  • Status can change: Category 1 is 'under evaluation'; FDA may move a substance to the 503A bulks list, Category 2 or off the lists. Re-check FDA's categories page (last updated May 14, 2026 as of October 2026).
  • Sterile compounding: injectables require sterile-compounding licensure; confirm state sterile permits and USP <797> BUD basis.
  • Cold chain: ask whether the product ships refrigerated/frozen and the pharmacy's stability data; NAD+ solutions are temperature sensitive.
  • API source and COA: NAD+ API quality varies; request the API supplier, identity/purity COA and finished-product potency testing.
  • Endotoxin testing: IV or large-volume vials need endotoxin and sterility results per lot.
  • No FDA-approved reference product: there is no approved NAD+ drug, so the essentially-a-copy test does not apply, but FDA has not reviewed the compounded product's safety or effectiveness.
  • State rules: some boards restrict IV compounded products for office use; check destination-state rules.

NAD+ compounding regulatory status (2026)

NAD+ (nicotinamide adenine dinucleotide) is not the active ingredient of any FDA-approved drug and, to AffinityRx's knowledge, has no USP-NF drug monograph (not confirmed; USP-NF is paywalled).

Status stated plainly: NAD+ is not an FDA-approved drug. FDA currently permits 503A pharmacies to compound it under the interim Category 1 (under evaluation) policy, which is provisional and can change. Compounded NAD+ is not reviewed by FDA for safety, effectiveness or quality.

FDA's 503A bulk drug substance categories list (updated May 14, 2026) places 'Nicotinamide Adenine Dinucleotide (NAD)' and its reduced form NADH disodium in Category 1 (under evaluation), which means FDA's interim policy lets 503A pharmacies compound with it under stated conditions while FDA evaluates it; it is not on the final 503A bulks list and is not on Category 2. It is not on a 503B list and there is no shortage, so 503B bulk compounding is not supported.

AffinityRx found no FDA-approved drug product with NAD+ as its active ingredient.

Frequently asked questions about compounded NAD+

How much does compounded NAD+ cost for clinics?
On AffinityRx, clinic prices for compounded NAD+ range from $10.00 to $165.00 across 3 listed strengths and fills. The lowest listed price is $10.00 for NAD+ (500 mg), 10 mL vial. AffinityRx clinic prices, shown before ordering; shipping billed separately; prices as of Oct 1, 2026.
Which pharmacies carry compounded NAD+ on AffinityRx?
11 pharmacies on AffinityRx list compounded NAD+: Emerald Pharmacy, Empower Pharmacy, HealthWealth Pharmacy, Perfect Rx, Promise Pharmacy, Rx Route, Striker Pharmacy, Strive Pharmacy, The Pharmacy Hub, Valiant Pharmacy, and Vault Pharmaceuticals. Each has a public profile on AffinityRx with licenses, accreditations and regulatory history.
How can a clinic pay less for compounded NAD+?
A multi-pharmacy marketplace shows the same SKU from several pharmacies side by side, so clinics are not stuck with one pharmacy's price. Clinics typically save about 10% versus their current price. Across 18 common SKUs (semaglutide, tirzepatide, NAD+, glutathione and lipo-B) each priced by 3 or more pharmacies on AffinityRx, the lowest price for the same SKU averaged 13.7% below the median pharmacy price (9.8% across the 13 GLP-1 SKUs). For NAD+ (1000 mg), 10 mL vial, the lowest of 10 pharmacy offers is 27.8% below the median pharmacy price. AffinityRx is that marketplace.
How fast does compounded NAD+ ship through AffinityRx, and what does shipping cost?
Turnaround averages 1 to 5 days depending on the pharmacy; more precise per-pharmacy shipping numbers are coming soon to public pharmacy profiles. Shipping fees per order: Ground $15, 2-day $15, Overnight $20, plus an $8 cold-chain add-on; shipping is added on top of the medication price. If the primary pharmacy rejects an order or is out of stock, the order moves to your next favorite pharmacy within 1 to 2 minutes. Prescriber re-sign is the default; turning on auto re-sign removes that step.
Can clinics order NAD+ through AffinityRx for office use or clinic stock?
Not available yet. Ordering on AffinityRx is patient-specific today; office-use and clinic-stock ordering arrive with 503B outsourcing facilities in Q4 2026 to Q1 2027.
Is NAD+ legal to compound in 2026?
Under FDA's interim policy, 503A pharmacies may compound with NAD (nicotinamide adenine dinucleotide) because FDA lists it in Category 1 (under evaluation), not Category 2. It is not on the final 503A bulks list and not FDA-approved as a drug, so this status is provisional and the compounded product is not FDA-reviewed.
Is NAD+ on FDA's 503A Category 2 safety-risk list?
No. FDA's 503A categories list updated May 14, 2026 shows NAD in Category 1 (under evaluation); Category 2 currently lists six substances, none of which is NAD+.
Is there an FDA-approved NAD+ drug?
No FDA-approved drug contains NAD+ as its active ingredient to AffinityRx's knowledge, . Compounded NAD+ is therefore not a copy of an approved product, but FDA has not evaluated its safety or effectiveness.
Can 503B outsourcing facilities supply NAD+ to clinics?
Not on the basis of any FDA list: NAD is not on the 503B bulks list and no shortage applies, so 503B bulk compounding of NAD+ is not supported by FDA's framework as of AffinityRx's October 2026 review. Clinics should use 503A patient-specific orders and confirm with the pharmacy.
What should a clinic ask a pharmacy before ordering injectable NAD+?
Ask for sterile-compounding licensure, API supplier and certificate of analysis, per-lot sterility, endotoxin and potency results, BUD basis, storage and shipping temperature, and written confirmation that the pharmacy compounds NAD+ under FDA's Category 1 interim policy.
